Choose the Method That Works for You
You can request your medical records through any of the following methods.
In Person
Visit the Medical Records department during regular business hours. A staff member will walk you through the request and verify your identity.
By Phone
Call Medical Records directly at (870) 816-3680. Have your photo ID and date of birth ready to verify your identity.
By Fax
Send your signed records request to (870) 816-3690.
By Mail
Mail your signed request to: Medical Records Department, Helena Hospital, 1801 Martin Luther King Jr Dr, Helena, AR 72342.
Fees
There is no fee for requesting your own personal medical records.
Processing Time
Most requests are fulfilled within 5 to 7 business days. Complex or archived requests may take longer — we’ll contact you if additional time is needed.
Prepare Your Request
Having these details ready will help us respond faster.
- Your full legal name (and any previous names used at the time of care)
- Date of birth
- Valid photo identification
- The dates of service or time period you’re requesting
- The type of records you need (full chart, test results, imaging, discharge summary, etc.)
- Where you’d like the records sent
- A signed release form (required for records sent to a third party)
Requesting Records for Someone Else
If you’re requesting records on behalf of another person, you’ll need documentation of your authorization to access those records.
- For a minor child: proof of parentage or legal guardianship
- For a patient who has authorized you: a signed HIPAA release form
- For a patient who cannot provide consent: power of attorney or court-appointed guardianship documentation
- For a deceased patient: documentation as the executor of the estate or next of kin
