- The employee’s name (or a description, if you don’t know their name)
- The department or area where you received care
- The approximate date of your visit
- What the employee did that made a difference to you
- Your name and contact information (optional, but helpful if we need to follow up)
Don’t worry about perfect wording — any recognition, big or small, is a gift to the team member who receives it.
